There’s a phrase practitioners hear so often it barely registers anymore.
A client, months or years into a serotonergic antidepressant, sits down and says some version of the same sentence: I’m not sad exactly. Things just have less sparkle.
Less sparkle. Less meaning. Less pull toward the people and pursuits that used to light them up. Apathy that arrived so gradually the client can’t say when it started.
In the previous post, Dr. Rabin drew the picture that explains this: the emotional window, narrowed from both ends at once. This clip is where he takes that picture and holds it up against the reports coming from real clients — and shows that the complaints practitioners hear most are not separate problems. They are one problem, expressed in different corners of a life.
The medication can’t tell the difference between suffering and joy. It turns the whole spectrum down together. So the states that sit near the top of the positive range, the peak experiences, the moments of deepest connection and delight, are precisely the ones that get clipped first. That’s why the loss shows up where it does — and Dr. Rabin names exactly where, in plain clinical terms, in the clip.
He’s equally clear on the other half: this is why these medications were never meant to be permanent. They stabilize, and stabilization matters. But a life with the top of the spectrum shaved off isn’t the destination. It’s the middle of a treatment that’s supposed to have an end.
